This form is for kittens under 6 months of age. If the kitten(s) you are looking to surrender is older than 6 months, please go back and fill out the ‘Cat Surrender Questionnaire’ form.
PLEASE READ BEFORE SUBMITTING THIS FORM:
Lake Humane Society is a managed intake facility, which means we do not euthanize healthy animals for time or space. All of our animals that come in, stay until they are adopted out to loving homes! Because of this, we almost always work off of a waitlist for new intakes and schedule surrender appointments based off of the amount of adoptions we go through in a day, on top of helping any animals our humane agent may bring in.
While we try to help any animal that needs to be surrendered, there are circumstances that the animal may not be a good fit at our facility. If we cannot accept it, we will try to provide any other resources we may have to help assist you.
PLEASE READ IF YOU ARE SURRENDERING STRAY KITTENS:
A kittens survival rate will be much better if they stay with their mom if they are under 6 weeks old (there will be an age chart in a question below to help you determine the kittens age!). Sometimes a mother cat will leave her kittens for hours on end to get food, look for a safer spot for her family, etc. Look at the kittens bellies, are they full? If so, it means mom is around and is feeding them, so please do not touch them and let them be!
Make sure if you are monitoring the kittens to see if mom returns to her babies, that you are doing it at a far distance so you don’t scare mom off! Frequently walking up to her kittens and checking on them can make the mom feel threatened and not want to come back.
PLEASE NOTE: Intakes are by appointment only.
