Youth Education Program Request Form – Instructions
Thank you for your interest in Lake Humane Society’s Youth Education Programs! Please read the following instructions carefully before completing the form:

Eligibility:
Our youth programs are free for all Lake County residents. A $35 fee applies for schools or groups located outside Lake County.

Program Selection:
Choose from the following:

  • Stay Safe Program includes certified bite prevention lessons to help children understand how to approach animals safely and responsibly—reducing risk of bites and improving outcomes for both kids and pets.
  • Be A Tree Program (Pre-K – 2nd Grade) – 30-minute session
  • Reach & Teach Humane Education Program (3rd – 7th Grade) – 60-minute session

Scheduling:

Programs are available Monday through Thursday between 9:30 AM and 2:00 PM.

Please select your preferred day(s) and time slot(s) based on your group’s needs and the selected program.

We recommend splitting larger groups into smaller ones to ensure the best experience for everyone.

Group Size & Age:
Indicate the total number of children, their grade level, and whether you will be splitting into groups.

Contact Information:
Provide a main point of contact for coordination and confirmation.

Confirmation:
Once your request is received, we will follow up to confirm availability and finalize your visit details.

Special Requests:
If you require a date outside our standard availability or have specific needs, please note them in the comments section—we’ll do our best to accommodate you!

Program Interest

Lake Humane Society offers free humane education programs for all Lake County residents (a $35 fee applies to groups outside of Lake County). Our goal is to promote safe, respectful interactions between children and animals, helping reduce fear-based incidents and animal surrenders.

Youth Education Program Age & Group Size

To ensure the best possible experience for both students and animals, we ask that you let us know how many children will be attending the Youth Education Program. If your group is larger than 15–20 children, we recommend splitting into smaller groups and rotating through activities. This allows for more hands-on interaction, better engagement, and a calmer environment for the animals. Please include the total number of children and chaperones when submitting your request so we can plan accordingly. We’re happy to help coordinate a schedule that works best for your group!

Scheduling Options for Youth Education Programs

Please select one of the following time options based on your group’s needs and the program selected. Please list your preferred day(s) and time slot(s) on the form. We will do our best to accommodate your request. Once submitted, we will follow up to confirm availability and finalize your visit details. Presentations are approximately one hour in length. If your group has specific scheduling needs, please include those in the comments section of the form.
